5 Jackson, MS Jackson, MS Full-time Full-time $20 an hour $20 an hour 7 days ago 7 days ago 7 days ago SCOPE AND PURPOSE Under the direction of the Asset Manager and working closely with the Property Manager, the Service Coordinator is responsible for the day to day identification of human service needs of residents of a housing facility, specifically related to issues of aging in place.
Identifies needs and develops service plans for the building.
Identifies available services in the community and works to make them available to the building, either on-site or through individual referrals to services.
Educates residents and other staff in service provision and issues of aging in place.
Assists the Manager in issues related to resident independence.
DUTIES AND RESPONSIBILITIES Research and maintain a network of service providers and eligibility.
Maintain a service directory of local resources.
Interact with local agencies and service providers in the community.
Act as liaison between management and service agencies in the community; Establish and maintain good relationships with residents; Coordinate the annual building assessment to identify residents who are frail and/or at risk.
Assist residents in the development of service plans.
Provide referrals to service agencies and service providers to the residents and monitor service delivery; Provide associated reports to HUD or others as required in a timely manner; Maintain confidential resident files in secure area, including documentation of interactions, plans and service referrals; Work with the Property Manager and assist the resident and their family when it is necessary for a resident to permanently move a to a more appropriate setting; Facilitate educational programs for residents and family members related to health, wellness, safety, financial assistance, resident rights or other appropriate information; Reports all suspected abuse situations to the appropriate agency; Maintain current knowledge of federal and state programs for the elderly; Establish and maintain contacts and visibility in the community through professional affiliations, networking, meetings with advisory and/or advocacy groups and others; Represents the property in a positive and professional manner by maintaining positive relationships with residents, vendors and staff; Will perform other duties as assigned by Supervisor.

Must demonstrate good judgement in day to day operations and must be trustworthy, dependable and show initiative.
QUALIFICATIONS Must have at least two years' experience with service management, advocacy and problem-solving concerning aging.
A Bachelor's Degree in Social Work, Gerontology, Psychology, Counseling or Public Health is also preferred.
Must have a minimum of 36 training hours of classroom/seminar time on aging and/or disability issues or must be willing to complete these minimum training requirements within 12 months of initial hiring.
Must attend 12 hours of continuing education training annually and demonstrate working knowledge of elderly care and the process and issues of aging, including disability, mental health and abuse issues.
Should have knowledge of:
i) local service agencies and resources for the elderly, ii) eligibility and application requirements for entitlement programs for the elderly, iii) general HUD program requirements for 202s and PRACs and iv) Fair Housing laws and regulations.
Have an understanding of legal liability issues relating to providing service coordination.
Possess strong interpersonal and communications skills, including oral and written communications.
Must be flexible in dealing with others.
Computer literacy is required including Word, Excel and the American Association of Service Coordinator's computer program.
ESSENTIAL JOB FUNCTIONS The Service Coordinator must:
i) possess vision to read printed materials, smart phone screens, and computer screens; ii) be able to clearly communicate in person, over a telephone and in writing in English (ability to communicate in other languages is a plus); iii) be able to handle, feel, and reach; iv) possess strength, stamina and mobility to perform basic office functions; v) interact positively with residents; vi) be able to work in settings with continuous interruptions and background noises; vii) be able to file letters, correspondence, reports, etc.
in file cabinets drawers ranging from 1 foot to 7 foot high; viii) be able to work under stress of meeting demanding deadlines, changing priorities and conditions; ix) have the ability to keep calm and remain professional with residents and staff in high pressure and emergency situations; x) be proficient with word processing, spreadsheets, e-mail and the Microsoft Office software and able to perform basic mathematical computations; xi) be able to work extended hours of time viewing a computer video monitor and/or operating a keyboard and be able to have daily exposure to video terminals; xii) must be able to analyze and develop solutions to problems; xiii) maintain an organized office; xiv) maintain a courteous and professional attitude at all times; xv) be available outside of normal working hours for emergency calls as needed; xvi) be available to occasionally travel to other properties/locations for training or provide support; and xvii) be committed to excellence and personal integrity.

Previous experience in a similar role is preferred.
