As a Geriatric, Family Medicine, or Internal Medicine Physician, you'll have the opportunity to:
-Spend quality time with patients, digging into their health stories and truly understanding their needs inside and outside the exam room.
-Take a hands-on, team-based approach to care.
Work closely with nurses, social workers, pharmacists, and other providers to develop comprehensive care plans tailored to each patient.
-Thrive in our unique value-based care model that prioritizes quality of life and improved outcomes for seniors.
We go beyond the clinic walls to address social determinants of health.
-Enjoy strong physician leadership, mentoring, and a supportive work culture.
The role:
-Full-time outpatient only, no weekends or on-call-Patient panel of 12-15 per day-Emphasis on preventive care, chronic disease management, care coordination-Opportunity for leadership and developmentCenterWell Senior Primary Care is changing the landscape of senior care.
