Pharmacist (Facility Program Manager)

Coordination of all clinical and distributive functions for the IV Compounded Sterile Products program and Non-Sterile products program. Reviews and interprets providers' orders for accuracy, eligibility, availability, and therapeutic appropriateness; contacts providers and makes authorized changes in orders or prescriptions as required. Uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the day. Performs other data processing work as required (for example, printing IV active order lists, scheduled IV labels, etc.). Uses significant knowledge and understanding of the VISTA IV Admixture computer program, and is able to utilize management reports functions for various reports and AMIS information. Uses knowledge of the standard related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs). This includes, but is not limited to, the standards established by DEA, FDA, VA, State, and JC. Serves as final technical authority over all professional Pharmacy matters pertaining to the IV Compounded Sterile Products program and Non-Sterile products program in collaboration with Inpatient Pharmacy Supervisor. Develops and revises service or station policies to reflect current Practice and meet all facility needs and comply with established medical center, VISN, and central office requirements. Audits or conducts quality management monitoring as demonstrated by provision of comprehensive quality assurance monitoring reports. Prepares and presents criteria for medication use evaluation pertaining to IV Compounded Sterile Products program and Non-Sterile products program when requested. Gathers data for administrative reports pertaining to the IV Compounded Sterile Products program and Non-Sterile products program. Assists in preparing for special accreditation and reviews. Participates in at least one hospital committee assignment per year, or attends a professional meeting and presents a report to the facility's professional staff and documents committee contribution or impact of meeting. Serves as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians, students, and pharmacists of lower grade. Plans and directs training of pharmacists, technicians, and students in the IV Compounded Sterile Products program and Non-Sterile products program area. Works with the Inpatient Supervisor to provide annual competency assessment for all staff. Presents in-service education to other pharmacy and hospital staff to include students as requested on sterile technique, special procedures for handling chemotherapy drugs, computerized IV program, or the therapeutics of intravenous admixtures. Monitors intravenous solution orders for interactions, appropriateness, and incompatibility. Communicates with providers concerning appropriate medication therapy and solves technical problems independently. Serves as consultant on parenteral nutrition as relates to TPN formulations and preparation and is knowledgeable in the various aspects of nutrition. Communicates effectively with peers and subordinates even under the most stressful circumstances. Knows and follows administrative requirements of all pharmacy sections as outlined in policies and procedures, covering narcotic control, formulary, investigational drugs, and general procurement procedures, etc. Provides collateral documentation for appropriate tasks as required by Pharmacy policy and procedure. Maintains accuracy of the IV Infusion Pump Library in collaboration with physicians and other health care professionals, IV Admixture Staff, and Clinical Specialists. Provides patient care and/or patient interaction appropriate to the cognitive, physical, emotional, and chronological maturation needs of the patient group served; adult age 18-64, and older adult age 65 and older. Supervises all professional and non-professional staff assigned to IV Compounded Sterile Products program and specifically, IV Technician(s). Ensures that all procurement requisitions related to the IV Compounded Sterile Products program and Non-Sterile products program are properly prepared, submitted on schedule, and that adequate stocks are maintained, and monitored. Ensures timely distribution of information and/or policy and procedure changes to all staff. Develops requests for new equipment, minor construction projects, and staffing needs to submit to upper management. Ensures proper maintenance of equipment, furniture and accessories. Receives complaints and grievances associated with the IV Admixture Program. Ensures that employees are aware of and comply with safety rules and regulations, including use of personal protective equipment, reporting an accident promptly. Work Schedule:

Citizen of the United States (U.S.). (Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with 38 U.S.C. 7407(a)). Education. (a) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR (b) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE:
Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Loss of Licensure or Certification. Once licensed or certified, pharmacist must maintain a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Loss of licensure or certification will result in removal from the GS-0660 clinical pharmacist series and may result in termination of employment. English Language Proficiency. The Pharmacist (Facility Program Manager) must be proficient in spoken and written English, as required by 38 U.S.C. 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation). Experience:
Active membership in at least one local, state, or national pharmaceutical organization 1 year of sterile and non-sterile compounding experience 1 year of Hazardous drug procedure experience Basic Cardiac Life Support (BCLS) certification is desired. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Work may involve prolonged standing and/or walking. Work involves lifting of weights up to 40 pounds. Incumbent will be subject to bending and stooping. Transportation of goods to and from pharmacy will be required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
